(1) A licensee may surrender the licence by giving written notice (a surrender notice ) of the surrender to the commissioner.
(2) The surrender notice must be accompanied by—
(a) the licence; or
(b) if the licence has been lost, stolen or destroyed—a statement verifying that the licence has been lost, stolen or destroyed.
Note 1 If a form is approved under s 228 for this provision, the form must be used.
Note 2 It is an offence to make a false or misleading statement, give false or misleading information or produce a false or misleading document (see Criminal Code
, pt 3.4).
