WORK HEALTH AND SAFETY REGULATION 2011 - REG 320

A general construction induction training card must—

        (a)     state the following:

              (i)     that the card-holder has completed general construction induction training;

              (ii)     the name of the card-holder;

              (iii)     the date on which the card was issued;

              (iv)     a unique identifying number;

              (v)     that the card was issued in the ACT; and

        (b)     contain the card-holder's signature.
