1979 No. 140 FEDERAL COURT RULES - RULE 52.4

4. (1) Subject to sub-rule 9 (1), an application shall be made in or
substantially in the form numbered 54 in the First Schedule.

(2) An application shall be accompanied by an affidavit showing-

   (a)  the nature of the case;

   (b)  the questions involved; and

   (c)  the reasons why leave should be given.
