Discharge and other planning
25 Discharge and other planning
(1) An accredited medical practitioner must take all reasonably practicable
steps to ensure that the dependent person, and the primary carer of the
person, are consulted in relation to planning the person's discharge and any
later treatment or other action considered in relation to the person.
(2) In
planning the discharge of the person, and any later treatment or other action
considered in relation to the person, the accredited medical practitioner must
take all reasonably practicable steps to consult with agencies involved in
providing relevant services to the person, any primary carer of the person and
any dependant of the person.
(3) The accredited medical practitioner must
take all reasonably practicable steps to provide the person, and the person's
primary carer, with appropriate information about follow-up care.
