(1) An application for a licence must be—(a) made to the QBCC commissioner; and(b) in the approved form; and(c) accompanied by each of the following—(i) the application fee prescribed under a regulation;(ii) the licence fee for the licence prescribed under a regulation;(iii) evidence of the applicant’s identity;(iv) if the applicant is not a building certifier—a copy of the applicant’s qualification or statement of attainment for an approved training course;(v) if the applicant is not a building certifier and the commissioner has approved a pool safety inspector test—evidence that the applicant has passed the test;(vi) evidence that the applicant has the prescribed professional indemnity insurance.
(2) Information in the application must, if the approved form requires, be verified by a statutory declaration.
