CRIME AND CORRUPTION ACT 2001 - SECT 146U
Form of authority
146U Form of authority
(1) An authority must be— (a) in writing in the form decided by the
chairperson; and
(b) signed by the person granting it.
(2) An authority must
state all of the following— (a) the name of the person granting the
authority;
(b) the date of the authority;
(c) details of the assumed
identity under the authority;
(d) details of the evidence of the assumed
identity that may be acquired under the authority;
(e) the conditions, if
any, to which the authority is subject;
(f) why the authority is granted;
(g) if the authority relates to an authorised identity officer—the name of
the official;
(h) if the authority relates to an authorised civilian— (i)
the name of the authorised civilian; and
(ii) the name of the civilian’s
supervisor under the authority; and
(iii) the period, of not more than 3
months, for which the authority remains in force.
(3) The authority also must
state the following— (a) whether it authorises an application to the
independent member for a birth certificate approval;
(b) each issuing agency
to which a request may be made under section 146ZB .
