(1) An application for internal review of a decision must—(a) be in the approved form; and(b) include enough information to enable the chief executive to decide the application.
(2) The application must be made within 28 days after—(a) the day the person is given an information notice about the decision; or(b) if the person is not given an information notice about the decision—the day the person otherwise becomes aware of the decision.
(3) The chief executive may, at any time, extend the time for making the application.
(4) The application must not be dealt with by—(a) the person who made the decision; or(b) a person in a less senior office than the person who made the decision.
(5) Subsection (4) applies despite the Acts Interpretation Act 1954 , section 27A .
