(1)  If the Principal Official Visitor knows or reasonably suspects that this Act has been contravened in any material way, he or she is to report the matter to the Health Complaints Commissioner or Ombudsman (whichever seems more appropriate in the circumstances).

(2)  At any time, the Principal Official Visitor may give the responsible Minister, the Chief Psychiatrist or the controlling authority of an approved facility a private report on any matter related to the responsibilities or activities of Official Visitors.

(3)  The Principal Official Visitor is to –

(a) give the responsible Minister, after the end of each quarter of a calendar year, a written report on the activities of Official Visitors during the quarter; and

(b) give the responsible Minister such further or additional reports on matters of special concern related to Official Visitor functions as the Minister may request.

(4)  The Principal Official Visitor is to give a copy of a report prepared under subsection (3) to –

(a) the Chief Psychiatrist; and

(b) the controlling authority of the relevant facility or the person in charge of the premises from which the relevant services are being provided under this Act.
