Application by person born as a result of a donor treatment procedure or descendant for information about donor (1) A person who is or may have been born as the result of a donor treatment procedure or who is the descendant of such a person may, on attaining the age of 18 years, apply to the Authority for information required to be recorded in the central register about a donor whose gametes were used in the procedure or whose gametes were used to form an embryo used in the procedure, being either or both of the following- (a) information about the donor (other than information from which the donor will or may be identified); (b) information which will or may identify the donor. (2) An application under this section must- (a) be in writing; and (b) be in the prescribed form and accompanied by the prescribed fee.
