(1) The essential
parts of a claim shall be —
(a) the
surname and christian or given names in full of the claimant; and
(b) the
residence of the claimant; and
(c) the
date of birth of the claimant; and
(d) the
place of birth of the claimant; and
(e) the
usual signature of the claimant in his own handwriting.
(2) If the residence
of the claimant is within a local government district or townsite, the name of
the street and the number of the house, if numbered, shall be stated, and if
not numbered, such particulars shall be given as, in the opinion of the
enrolment officer, are sufficient to enable the exact locality of the
claimant’s residence to be ascertained.
(3) If the residence
of a claimant is not within a local government district or townsite, his
residence shall be stated with such particulars as are, in the opinion of the
enrolment officer, sufficient to enable the exact locality of the
claimant’s residence to be ascertained.
(4) If the claimant is
unable to insert in his claim the date of his birth because it is not known to
him but —
(a) the
claim contains a statement to that effect; and
(b) a
person who may attest as to the claimant’s identity under the
Commonwealth Electoral Act 1918 section 98AA(2)(c), as read with
section 42(1)(ca), certifies in writing that he or she is satisfied that the
claimant is not under 18 years of age,
the claim shall be
taken to sufficiently comply with subsection (1)(c).
