(1) A worker may request the employer of the worker to supply the following information--(a) the employer's name and address for the service of documents for the purposes of this Act, and(b) the name and address of the insurer from whom the employer has obtained a policy of insurance or, if the employer is a self-insurer, to be so informed.
(2) An employer, or a person acting for an employer in the management of the employer's trade or business, must not--(a) fail to supply any such information, or(b) supply information which the employer or person knows to be false or misleading in a material particular.: Maximum penalty--20 penalty units.
(3) In this section--
"employer" , in relation to a worker, includes a principal within the meaning of section 20 of the 1987 Act who is liable to pay compensation to the worker.
