Form of certificate of competency

16. Form of certificate of competency



A certificate of competency must be in the form determined by the Authority
and must include-

   (a)  the name and signature of the certificate holder; and

   (b)  the date of issue; and

   (c)  the class of certificate; and

   (d)  a description of the type of work that the certificate authorises the
        certificate holder to carry out; and

   (e)  an identifying number unique to the certificate holder.
