Replacement of lost, stolen or destroyed certificates

31. Replacement of lost, stolen or destroyed certificates



(1) If a certificate of competency is lost, stolen or destroyed, the person to
whom the certificate was issued may apply to the Authority for a replacement
certificate.

(2) The application must be in the form determined by the Authority and must
be accompanied by a fee of $23.50.





(3) The Authority may issue a replacement certificate to an applicant if the
Authority is satisfied that the applicant's certificate has been lost, stolen
or destroyed.

(4) If the Authority refuses to issue a replacement certificate to an
applicant, the Authority must give the applicant a written notice of refusal
that sets out the reasons why the Authority is not satisfied that the
applicant's certificate has been lost, stolen or destroyed.
